In today's overly litigious society, it's safer for them to write a drug approved by the FDA for the disorder rather than writing it off label, for starters.

There are very few identical generics. Only if the original manufacturer makes them are they identical and as pure, effective and safe.

Psychological reasons. Many people are about brands rather than copycats. If you are willing to pay extra, so be it. God bless America and the right to choose!!!

 

I will sell pristique the best I can. I am still learning about it and am not really impressed with it. Maybe I will see a study that will change my mind. It looks like its side effect profile is pretty lengthy. I hope I misread because I really want it to do well for all of us that were retained.

 

As someone who is close to the Medical Affair, I hate to burst your bubble. Pristiq is not a good drug by any standard. We tried to get 100mg approved as the standard dose. But our patients got so sick that they could care less about the efficacy. They just couldn't tolerate the drug long enough to see any improvement. We were then forced to compromise the efficacy and tested the 50mg and hoped for the best. As far as we know, 50mg is about as effective as 100mg XR with no additional safety or novel features. So if you want to convert your current XR patients to Pristiq, you may not want those who are on 150mg XR and higher or your customers will be very disappointed. As an extension, we consider it as a failure. Our company may not want us know about this or to think this way as we can expect. We can only rely on marketing now for any hope as there is nothing else we can do on this end. Pharmacologically, we are very disappointed and it is certainly an inferior product.

You'd better hope that Pristiq does a billion or there will be another round of layoffs mid-year I'd say in order to get to the 10% total company cut figure that was announced. I do NOT believe Pristiq will do well. Too much competition now and too much similarity to Effexor. Psych patients are on multiple drugs and will look to save a buck with generic effex. Read all about the Pristiq hot flash study done in Oregon on PubMed. HUGE dropout rate rendering any results meaningless. The product is one more cruel joke on women. Wyeth hates women and sees them only as exploitable cash cows. Really awful.

Stress reduction is key in controlling hot flashes. Drugging women into chemical hazes as a substitute for life and attitude changes is reprehensible. Deep breathing controls hot flashes just as much as any harmful, side effect riddled drug.

You are obviously not a Wyeth Pharma person, or Wyeth employee, at all. You're busted. You can say a lot of bad things about how upper management treated the sales force during this RIF, but one thing that's NOT true in the slightest is that Wyeth "pays" doctors. As a matter of fact, Wyeth is without a doubt the most conservative big pharma company when it comes to entertainment or anything like that. They just don't do it. Period.

The reason Lexapro is doing and has been doing so well is it's tolerability profile.

The most important thing when developing an antidepressant is tolerability first, efficacy second, then safety. Lexapro is a week SSRI. but what about the 35% remission rates you get with a placebo? It goes to show you that the patients want to be told "This pill will make you feel better and you should not have the high rates of nausea, or discontinuation syndrome you would probably have with second line agents."

The reason why Cymbalta has been doing well, is because of their marketing for DNP. Docs are writing that stuff in combo with Lyrica. Or they are just treating folks with it that come in complaining of unexplained vague aches and pain (somatic stuff).

Pristiq better have a niche, or be better tolerated than XR. If we don't get the vaso indication, then the company better reduce its expecations on this product.

 

I read an article regarding Pristiq in which it was explained that the "des" means a subtraction of part of the original venlafaxine molecule. In fact, the new formulation will be better tolerated as the liver won't have to metabolize as much molecule as with EXR. Better tolerability is really the bottom line with Pristiq and I think we will have that as part of our detail pieces (I hope).

 

We will just have to wait and see- Lunesta is a great sleep aid- but insurance won't cover it due to generic Ambien-Pts are not going to pay 150.00 out of their pocket for this product. Their market share stinks. Most of the time generic Ambien works. No matter how much marketing P did- Exubera failed in the market place. I do think that if Wyeth doesn't advertise on TV- they will be hurt. After the Vytorin mess, many doctors are sticking with what they know and what is generic. They are very skeptical of what a rep tells them. Avandia is also a good example.
Pristiq can be huge or fizzle- we will have to wait to see insurance coverage and how patients react to it- will it be different than how they respond to what is out there now. Let's be honest- we are reps, not pharmacologists, we only know what the company wants us to know. I have been burned too many times by my company not telling the whole story to believe everything a company tells me.

 

PP I think you are correct--we will have to wait and see where we are on the MC plans...tier 2 we will be golden but if tier 3 it will be a "fizzle"...pts are too cost sensitive in this economy.

 

With Pristiq we are reaching for straws. W/out vasomotor indication this drug will wimper to market - not roar!

I think even with the vasomotor indication Pristiq might not take off. Women are taking control of their own bodies and I don't know if may women want the side effects of an antidepressant along with menapause.
