    Wasn’t the Hershey case supposed to be a slam dunk, and the first in a chain reaction of judgements that was going to destroy Intuitive???

    why the interest with this case?

    on a different note, curious why so many of the new adverse events associated with injury and death have thrown away the reusable instruments so failure analysis cannot be done?

    don’t worry about my background knowledge. I have read the pages and probably wrote a good % of what is left of DWAS.

    I see the FDA has yet to dismiss the citizens petition advising the ban/recall of the monopolar cautery instruments, but so many of the new injuries and the two deaths seem connected to the design defects explained in the petition. The petition seems to be “sticking” so guessing there must be at a minimum “something “ that is keeping it alive, unlike the victims listed in Maude this month.

    It’s the responsibility of the institution to quarantine any device involved in an adverse event, so maybe you should direct your question to them.

    It’s the responsibility of the manufacturer to follow ISO standards in their manufacturing plants too. Read the petition. That is what it is referencing.

    ISI has never fixed their design defects or the manufacturing inconsistencies. This has been going on since day 1. The FDA nailed them years ago for exactly the same issues.

    “Design defects were also noted with certain surgical instruments...led to tears or holes in protective tip covers that led to arcing that in turn led to injuries to patients.”

    Don’t you understand? It’s a giant conspiracy to harm patients and the hospitals are totally in on it! No one can be trusted. Except DWAS.

    this is so sad. The design defects are not fixed. 20% of last months reported adverse events were due to arcing.

    The ISI CSRs have been busy little elves I see. Something got under their skin... no holiday bonus this year losers? I wish you weren't so quick to throw out uncited statements that have no relevance or even truth to them. Here is the CITED documentation from your own employer.

    How does monitoring "prevent" capacitive coupling? In a perfect world it would detect it. Please provide the technical specifics as they relate to the davinci robot's model to back up this unfounded statement.

    The FDA has confirmed historically that the manufacturer has violated Federal law as it relates to adverse reporting of the DaVinci robot.     “Broken wire at the weld combined with silicone potting separation likely created a path for arcing to occur, which confirms the customer reported complaint. Wire breakage at weld is a known issue which is not attributed to mishandling/misuse engineering has determined that the failure related to this complaint, the conductor wire pulling out from the weld location at the base of the instrument, is not user interaction related.“

    Description of Patent : Some surgical tools that utilize electrical power are susceptible to capacitive coupling with other surgical tools, such as other instruments, cannulas, etc. during use. Capacitive coupling can generate a leakage current in the surgical tool which, if not dissipated in a controlled and predictable manner, could potentially result in misdirection of electrical energy. In some cases, the cannula is made from a conductive material, such as a metal or alloy, and contact or sufficient proximity between the conductive cannula and a conductive portion of the instrument permits a conductive pathway from the instrument to the cannula to dissipate any leakage current to the patient's body wall through the outer surface of the conductive cannula, with the patient typically being further electrically grounded via a return pad to a ground terminal. A need exists to provide a conductive path between a conductive cannula and an instrument that is inserted through a reducer device positioned within the cannula. It is desirable to provide such a conductive path while providing reducer devices that are relatively inexpensive to manufacture.
