With the tremendous pressure to show these duds are being presented, it has now become critical to find customers willing to allow them, but customers who can't hurt me when they walk away mad after my DBM puts them in the death lock and "asks for the business." If I have someone who really counts, there's no way I am taking the risk.
Just do them with pharmacists. Pharmacists hate Pfizer and aren't going to increase Zyvox use regardless of what the data say. Your DBM can grapple all they want with the pharmacist and it won't change a thing. You can always do them with IDs as well because they don't care - they can write anything they want. IDs hate DBMs almost as much as they hate us. Just keep them away from ED and Hospitalists who actually write all the orders.
