It all depends on your states insurance coverage. Most reps are dealing with insurance coverage that has a high copay for patients or its not covered at all. Also PPI's are not recommended first line. Patients may have to fail on something else before docs will write it. The bottom line is docs don't really want to use it because it can be expensive, patients may have to fail on other meds before it would be covered under their insurance and docs don't want to deal with prior authorization which takes time if it's not covered under insurance.
