How are Managers at Pfizer compensated?

As long as Pfizer continues to put primary care leaders and mindsets into non-primary care businesses, you should expect simple, non-applicable metrics to be used. They will not try to understand a different business, they will not listen to those hired or acquired who do understand the business and will revert to primary care metrics. Even my local "KAM" has metrics they must follow even it does not support business growth from reps in the district.
