    Oh, how the mighty have fallen. A once lucrative respiratory franchise has now turned to dust (not turned to the mist). My guess is Spiriva would have been a bust too if not for Pfizer.

    But an ICS isn't necessary. We can totally change COPD care just like Pfizer did (I mean us) with Spiriva so long ago!

    It's really simple, there was nothing before Spiriva. It was the first of it's kind. A true, LAMA. It was needed. It was very much wanted. Stiolto isn't wanted. Stiolto isn't needed. Stiolto isn't even close to the first of it's kind. Trelegy is the frist of it's kind. Granted it's not super needed, Dr.s can just use SpV and Advair/Symbicort, but it'll be nice for Dr.s to have. The limited indication means nothing. Dr.s write whatever they want, however they want. BI missed it's chance, GSK's turn to rake in the profits. Goodbye respitory dept. when SpV goes generic.

    Why would I train them on multiple devices with one already being a pain. One device ANY combination you see fit not what we tell you you should be doing

    It is simple, BI bet on the device. I was told that the reason they didn't bring out a triple is because the particle size can't fit through the device. When Spiriva was launched in 2004 there was talk about BI coming out with a triple in the years to come. Someone made the decision it wasn't needed and the device would ultimately win the day....these are the decisions that make or break a company. It is unfortunate, I have had a long career with BI and it will likely come to an end because of this decision and others like them.
