Phlebotomy Tech Certification Completion

Hello Quest Community,

I live in and work in TX. In the process of relocating back to CA
and need the Phlebotomy Tech Certification Qualification by Experience
form to be completed in order to take NCCT.

Form has been sent to supervisor, manager & hr. No one knows
who should complete. It is to be completed by Lab Manager but
I work at a Family Med Ctr not owned by Quest. I am presently
waiting for a return phone call from Talent Acquisition to see if
they have another suggestion of who can complete because they
initially told me to contact hr.

ANY SUGGESTIONS?

 

I’m not sure if you are serious but if you really are looking for answers of this board, you are in trouble.

 

Knowing how the red tape at Quest works, your best bet is to fill it out and sign it yourself/
