    I've been an advocate of Novartis products when reasonable in practice. Price it so plans cover it, as I don't see enough here to justify a premium.

    (Reference, Medscape) Among the top reservations about PARADIGM-HF is the extent to which it should replace ACE inhibitors in patients with systolic heart failure, for whom the agents are a mainstay. The trial randomized only patients who successfully completed a single-blind run-in phase consisting of enalapril for two weeks and then LCZ699 for twice that long—all after being stable for at least a month on either an ACE inhibitor or angiotensin-receptor blocker (ARB). Of the 10 521 entering the run-in phase, >2000 dropped out, mostly because they couldn't tolerate one of the two agents, often because of hypotension; ultimately 8442 were randomized.

    "How do we know what will happen in the real world?" Konstam asked. The study doesn't show how many patients never before exposed to the drug would become hypotensive. In clinical practice, he said, "You don't get a run-in period."

    the clinical trial took place in the real world as far as i'm concerned.
    to be honest, lot of hype around the LCZ, but name any new (block-buster potential) product that wasn't.

    WE WORK FOR BIG PHARMA!!!!

    WHY WOULD YOU EXPECT ANYTHING LESS???

    Reading almost every post about LCZ696 and anytime there is criticism that it might not be the most amazing revolutionary product ever to come out in the history of the world, the poster is called a troll or said that they must be a competitor or disgruntled ex employee.
    Better get thicker skin out in the field because the docs aren't going to write just because of hype and they are going question everything about this including the study design, price, etc.

    Doctor we took the highest dose of diovan added an unproven NI that may contribute to acceleration of alzheimers & compared it to a modest dose of the oldest acei in existent.
    Here is your honararium. Can you make sure your next patients pay 100x the cost of a generic Ace/Arb & Diuretic (that will probably work better) ?

    It could be the best drug in the world but if patients can't afford it then none of it matters. Period. They will not price it right and then expect us to fight bears with a stick. The best salesperson in the universe can't do anything if the drug is not accessible to patients. Period.

    I'm not sure why everyone didn't see this coming? Of course physicians are going to say this as it's a very accurate statement of their reality. We can talk readmissions all day long but there is no way to tell if the results would have been the same if the study was done with the comparable group being high dose ACE or ACE, ARB diuretic. Remember that ValHeft showed similar results..so why can't the Ace/Arb& diuretic do the same?

    LMAO, as you are so worried about this and that, the competion is already positioning LCZ ... And as they should as competent sales people. That is if you don't position your product in the market place first then others will. So all the " heart failure education" b.s is really a waste of your time. Have you postitioned your product or are you letting your competition do it for you. Are your managers and directors savvy enough to know how to coach and teach on how to position a product before a launch or are they just doing CPT and QTQ b.s. And making sure your expense report is in compliance. My sense is that it is the latter. Time to step it up if you want this thing to be successful. Don't reguritate the company B.S. The customer can see though it like a laser. You will be best served in positioning the product now or forever hold your peace as the competition has done their job.

As far as Medicine doesn't sell like widgets ect... You are either very naive, ignorant or simply stupid. Hopefully the first two cause as they say you can't fix stupid.

    other regulations as well as a history of bogus studies & multi million $ settlements of the FCA to name a few
