Why would insiders be selling stock in Oct.-Nov. of this year? I am not clear if that is a good sign or a bad sign.

Insiders are required to submit to proposed sale to the govt. prior to transactions. These trades were probably decided 3 months ago.

End of year, so maybe the trades are due to tax ramifications? Or the stock is trading at or near a 52 week high so they're doing what we all should be doing...rebalancing, taking profits, reducing exposure to one business sector.

Or if this makes you feel better, the rats are abandoning the ship! Pf is about to announce a (insert doom scenario) Layoffs, new govt. settlement for off label promotion, new lawsuits for key drugs...whatever

Your first two points are probably the reason. The last point I believe is illegal, since that would be trading based on inside information.
