So, what caused it? Was it a defect in the instrument or something else?
“The scrub tech informed the csr that he believes that there were possible instrument collisions during the case.”
Sad to say but these things happen. Not a reflection on the device! But on how the device is used. Sometimes best to keep your mouth shut until you have all the facts. This horse has been beaten or years and never gained any traction. Sit back down and wait for the facts before you stir up shit. Did it work the first time probably won’t work this time. Just ask Albert. I’m sure he’s happy with the results of all his he’ll raising.

what is it that is or isn’t “working?” Gaining traction would mean fewer manufacturing anomalies and decreased recalls based on nonconforming materials from lack of Following ISO processes. The fact you seem to think the heart of the issue is money for the survivors shows your “brokenness” around how you feel about mankind in general. I feel sad for you. A true dwas survivor wants to see future patients with fewer complications, injuries and deaths. Lawsuits result from victims complaints not getting the attention they deserve. A true Silicon Valley technology company would understand this and embrace customer/patient feedback as an opportunity for a better product, innovation potential, and disruption of a current market they are already losing due to the absence of machine learning, analytics and root cause analysis.
