Was it Joe that:

launched without medical letters?

delayed trial recruitment for over 2 years?

focused on paying reps for sampling as many doctors as possible?

created a "sell immediately or be fired" atmosphere that forced bad reps into making bad decisions and good reps into leaving?

failed to put together effective ad boards? (this one was his fault)

entrusted daily field operations to a group of inexperienced, unethical, simpletons (RBDs
