3. Rare and serious disease = low volume of patients. If volume of patients higher could charge less. Economics 101

5. Does cost money to promote it

It’s only used on a subset of Rheum patients and other difficult to treat patients. If it were used commonly, it would likely cost less. Again - a price x volume equation.
