I am aware of an instance where a lab appears to be dramatically overcharging for its services. However, there are stories in local media that an insurance company is saying the lab has overcharged them multiple millions of dollars.

If no case has been claiming fraud against the government has been filed though, can a Qui Tam case still be filed?
